Scope of Appointment Guidelines Forms

UnitedHealthcare January Training for Arizona Agents

According to the updated Medicare Marketing Guidelines, agents must obtain Scope of Appointment forms “48-hours in advance whenever practical.”  This information can be found in Chapter 3, Section 70.10.3 of the Medicare Marketing Guidelines.  Practical reasons may include:

  • Walk-In Appointments
  • The end of AEP
  • Surprise guest at prescheduled in-home appointments

Agents should always note the reason the Scope of Appointment form was not signed at least 48 hours prior to the appointment on the bottom of the form.  As in the past, these forms must be retained on file for a minimum of 10 years regardless of whether the appointment resulted in a sell.
